Support coordination and case management services may be funded through various government systems including the NDIS. Our staff take a holistic and person-centered approach, identifying tailored and specific supports that assist you to achieve your best outcomes. Our team also provide group training and information sessions to help you understand how to use your plan more effectively, transitioning you to independence and autonomy in using your plan.
Allied Health
Our allied health staff are a dedicated team of experienced practitioners that enable you to lead a better life. Some therapies may assist with speech, mobility, mental well-being, or a combination of supports to address multiple needs.
Our people help you to understand what type of supports you require to build your independence and assist with implementing a plan that best suits your needs. We provide assessments, exercise programs, and plans to assist with everyday tasks.
Community Supports
Our team provide innovative and exciting ways to build your independence. We provide support for community and in-home activities that assist in achieving your goals, using industry leading methods and accountability standards to build your independence and get the best out of your funded supports. We offer individual, and group based, supports across WA.
Employment and SLES
Our skilled team of Employment Specialists support you to achieve your employment goals, whether in open employment or through building your own business. These supports assist you to build long term skills to achieve sustainable independence in the workplace or self-employment. Our specialised staff also provide School Leaver Employment Supports, supporting young people through the Discovery process, to prepare them for the workplace.
